About the Opportunity:
Reporting to the Manager, FCSS, the Family Home Visitor Coordinator will provide supervision and direction to the Family Resource Network Team in addition to maintaining a moderate case load of home visitation and family support clients. With the overall goal to strengthen and build our communities’ children, youth and families, this new program will serve the full spectrum of demographics (0-18) with a focus on providing a full continuum of preventative support services for the family as a whole. This will include universal, targeted and intensive programming via direct home visitation, family supports, caregiver capacity, social connections, and child development and well-being opportunities.
Responsibilities of the position include, but are not limited to:
- Lead the implementation of home visitation best practices approaches and curriculums
- Develop partnerships and maintain ongoing contact with social services and other supportive agencies
- Represent the City of Leduc on committees/coalitions as required
- Participate in community, FRN and FCSS events to promote this and other program and services
- Identify and support individual’s at-risk and needing intense and targeted support and provide weekly visits
- Work collaboratively with other departments and agencies to support resident in achieving goals/objectives
- Assist and support the resident to navigate and access services and resources (eg. Subsidized programs, health care, etc.)
- Respond to referrals for families from other agencies and FCSS
- Complete relevant documentation and maintain records of all activities including assessment records, contact notes and referrals
- Adhere to FRN yearly budget, and provide necessary date for reporting and ensure program outcomes are met through evaluation tools
